چکیده

Introduction: Electrical storm is commonly defined as the occurrence of three or more episodes of ventricular tachycardia (VT) or ventricular fibrillation (VF) in 24 hours. Patients with an implantable cardiac defibrillator (ICD) are at increased risk of electrical storm due to history of decreased ejection fraction and/or sustained VT/VF. In addition to medical treatment of electrical storm in ICD patients, special consideration should be given to ICD reprogramming to optimize hemodynamics by increasing basic pacing rate and maintaining atrioventricular as well as interventricular synchrony. If possible, anti-tachycardia pacing rather than repeated shocks can reduce sympathetic tone. Unnecessary right ventricular pacing may worsen left ventricular function by desynchronizing the ventricles and is generally avoided. Case Report: We present a case of a 67-year-old white male with severe electrical storm due to polymorphic ventricular tachycardia (PMVT) that was dramatically brought under immediate control by forced right ventricular pacing. Subsequent continuous right ventricular pacing helped suppress any ventricular tachycardia recurrence until catheter ablation was performed eight months later. Conclusion: When confronted with patients with refractory electrical storm, we propose programming a faster right ventricular pacing rate. (This page in not part of the published article.) Temporal Trends and Temperature-Related Incidence of Electrical Storm

Electrical storm (ES) is usually defined as the occurrence of ≥3 episodes of sustained ventricular tachycardia (VT) or ventricular fibrillation (VF) within 24 hours requiring either antitachycardia pacing or cardioversion/defibrillation. ES represents a true medical emergency that requires a multidisciplinary approach, and its prevalence is steadily rising along with the number of patients bear...

Idiopathic ventricular fibrillation triggered by two distinct foci.

Idiopathic ventricular fibrillation (VF) is often triggered by short coupled ventricular premature beats originating from the ventricular Purkinje system or from the myocardium of the right ventricular outflow tract. We present a case of electrical storm due to idiopathic VF triggered from two distinct arrhythmogenic foci within the right ventricle. Successful radiofrequency ablation of both fo...

Successful use of quinidine in treatment of electrical storm in Brugada syndrome.

We report an adolescent with a malignant form of Brugada syndrome who presented with 15 episodes of ventricular fibrillation (VF) over 10 days, shortly after implantation of an implantable cardioverter defibrillator. Oral quinidine bisulphate at a dose of 1000 mg/day successfully suppressed the electrical storm and recurrence of VF over 18-month follow-up. It also normalized the ST-segment elev...

An inappropriate pacing threshold increase after repeated electrical storm in a patient with implantable cardioverter defibrillator

BACKGROUND Implantable cardioverter defibrillators (ICD) are capable of effectively terminating malignant ventricular arrhythmia and are the most effective way to prevent sudden cardiac death. However, some evidences demonstrated that both anti-tachycardia pacing (ATP) and ICD shock can also bring adverse prognosis. CASE PRESENTATION A 66-year-old Han Chinese man with prior ICD implantation w...

Calming The Storm By Sympatholysis

An electrical storm is a fearful and life threatening experience for the patient while it is a therapeutic challenge of the extreme degree for the physician. In this issue of the Journal, Proietti and Sagone have presented an in depth discussion on the various aspects of the electrical storm including the current day management [1]. As rightly mentioned by them, the present definition of an ele...
